Referring Someone to the Housing First Program
Tenants accepted into Cobblegate, Tangy Court or Belvo must meet certain criteria.
A prospective Cobblegate tenant must:
- Meet the U.S. Department of Housing and Urban Development (HUD) definition of homelessness (preference is given to individuals who are chronically homeless)
- Provide a diagnosis consistent with a serious mental illness as defined by HUD
- Be currently unlinked to public mental health services
- Need the level of services provided by the Housing First Program as determined by the Homeless Solutions Policy Board’s centralized assessment process
- Be willing to sign and comply with the terms of a standard lease
A prospective Tangy Court tenant must:
- Meet the HUD definition of chronic homelessness
- Provide a diagnosis consistent with a serious mental illness from Samaritan CrisisCare or the Samaritan Homeless Clinic
- Be linked with a community psychiatric support agency (funded by the Alcohol, Drug Addiction and Mental Health Services [ADAMHS] Board for Montgomery County) and receiving community psychiatric support services
- Need the level of services provided by the Housing First Program as determined by the Homeless Solutions Policy Board’s centralized assessment process
- Be able to sign and comply with the terms of a standard lease
A prospective Belvo tenant must:
- Meet the HUD definition of chronic homelessness
- Be unsheltered, living in a place not meant for habitation (in a park, in a car or under a bridge)
- Provide a diagnosis consistent with a serious mental illness
- Need the level of services provided by the Housing First Program as determined by the Homeless Solutions Policy Board’s centralized assessment process
- Be able to sign and comply with the terms of a standard lease
Documentation required for referrals
- Most recent biopsychosocial assessment
- List of current medications
- Proof of legal status
- A completed Authorization for Release of Information
- A copy of the prospective client’s most recent Individual Client Plan
- The client’s social security card
- The client’s birth certificate
- Proof of insurance (if any)
- Proof of residency (Montgomery County)
- Proof of income (if any)
