Referring Someone for Residential Adult Care Facilities
PLACES has adopted the admission criteria of the Alcohol, Drug Addiction and Mental Health Services (ADAMHS) Board for Montgomery County.
A potential resident must:
- Be at least 18 years old
- Be a Montgomery County resident
- Meet the Ohio Department of Mental Health’s definition of severely and persistently mentally ill
- Be linked with an ADAMHS Board contract agency:
- Consumer Advocacy Model (CAM) program at the Boonshoft School of Medicine at Wright State University
- Day-Mont Behavioral Health Care
- Eastway Behavioral Healthcare Corporation
- South Community Behavioral Healthcare
- Have a primary Community Psychiatric Support Specialist
- Be able to sign a resident agreement (or have a guardian willing to sign)
- Meet ADAMHS Board criteria for residential care
Individuals with histories of violence, substance abuse or both are considered on a case-by-case basis.
Documentation required for referrals:
- Most recent biopsychosocial assessment
- List of current medications
- Proof of legal status
- A completed Authorization for Release of Information
- A copy of the prospective client’s most recent Individual Client Plan
- The client’s social security card
- The client’s birth certificate
- Proof of insurance (if any)
- Proof of residency (Montgomery County)
- Proof of income (if any)
