Referring Someone for Residential Adult Care Facilities


PLACES has adopted the admission criteria of the Alcohol, Drug Addiction and Mental Health Services (ADAMHS) Board for Montgomery County.
A potential resident must:
- Be at least 18 years old
- Be a Montgomery County resident
- Have a severe and persistent mental illness diagnosis as defined by the Ohio Department of Mental Health and Addiction Services
- Be linked with an ADAMHS Board community mental health center:
- Consumer Advocacy Model (CAM) program at Samaritan Behavioral Health, Inc.
- Eastway Behavioral Healthcare Corporation
- South Community Behavioral Healthcare
- Have a primary Community Psychiatric Support Specialist
- Be able to sign a resident agreement
- Meet ADAMHS Board criteria for residential care
Individuals with histories of violence, substance abuse or both are considered on a case-by-case basis.
Documentation required for referrals:
- Most recent biopsychosocial/diagnostic assessment
- List of current medications
- A completed Authorization for Release of Information
- Most recent Individual Service Plan
- Social Security card
- Birth certificate
- Proof of medical insurance
- Proof of residency (Montgomery County)
- Proof of income
- Completed housing referral
Referral process:
- An individual’s Community Psychiatric Support Specialist must submit a housing referral to the Eastway Residential Support Housing Wait List.
- Priority is given to individuals being discharged from statewide behavioral healthcare facilities and hospitals.
